Due to second semester worksite restrictions, this class has limited enrollment and an application process. Pick-up a Health Occupations Application Packet from Mrs. Lemerande in room S-2 or from your counselor. In the packet you will be given instructions to do the following:

1. Complete the one page "Student Application Form".
2. Prepare and submit a resume. Use the resume outline provided as a guide only. This must be typed or neatly written in ink.
3. Ask the attendance office to complete the "Attendance Record Form". Please allow at least three days for this to be completed. The attendance office will return this form to Mrs. Lemerande.
4. Complete the "Unofficial Transcript Request" and give it to Mrs. Bridenbeck in the office. Please allow at least three days for this to be completed. Mrs. Bridenbeck will give the transcript directly to Mrs. Lemerande.
5. Ask one science teacher (besides Mrs. Lemerande) to complete the "Science Teacher Reference Form". This science teacher will give the form directly to Mrs. Lemerande.
6. Ask one person of choice (counselor, non-science teacher, or administrator) to complete the "Teacher Reference Form". This person will return the form directly to Mrs. Lemerande.
7. You are to read and sign the "Student Responsibility Agreement Form".
8. Have a parent or parents read and sign the "Parent Responsibility Agreement Form".
9. You are to turn in the following directly to Mrs. Lemerande in room S-2:
  • Student Application for Health Occupations
  • Resume
  • Student Responsibility Agreement
  • Parent Responsibility Agreement

It is your responsibility to check with Mrs. Lemerande to make sure your application has been turned in and is complete. Any questions??? See Mrs. Lemerande in room S-2. The entire application packet is due by the end of the second week in April.
